Eight years ago, when the first season of Game of Thrones was released, there were several unforgettable scenes. One of these scenes featured a row of heads on spikes, meant to show the public what happened to "traitors" who were executed.

Originally, the producers planned to have 20 custom-made heads designed for the scene. But the cost was too high, especially since the show was working with a limited budget for its first season.
So, the production team found a reseller selling a box of heads. When the box arrived, they mounted them on spikes. However, one of the heads turned out to be of someone unexpected.
They had a tight production schedule, so they quickly put a wig on the head and filmed it without showing the face for too long.
The show was released without anyone mentioning the mistake. But later, during a DVD commentary, one of the producers accidentally revealed that it was the head of George W. Bush. This caused a lot of controversy with conservative groups, who criticized the show.
The producers and HBO explained that it wasn’t meant to be a political statement — they just had to use whatever they could get. HBO issued an apology, and the situation eventually blew over.

But I’m sure they learned a valuable lesson: always be careful about what you say in interviews. Political media can turn anything into a controversy, and they’re always looking for a "scandal."
